Overview:This document describes how to prepare a file for submission to the National Strategic Tracing Service (NSTS) batch tracing service.  
Abstract:This document describes how to prepare a file for submission to the National Strategic Tracing Service (NSTS) batch tracing service. This service should be used prior to mailing questionnaires, in order to identify any patients who have died since they were discharged from hospital. NSTS is routinely used by a registered member of NHS Trust staff (the delegated authority and batch trace user) for checking NHS numbers and other patient details. This Instruction Manual has been produced by SchlumbergerSema to describe how to use the NSTS for the purpose of checking for deceased patients, prior to mailing questionnaires. Registered batch trace users are advised to follow these instructions carefully, to ensure that the file they submit conforms to the file specification required by SchlumbergerSema, in order to avoid any delays in processing.This document gives full details of the data items which should be included in a ‘Patient Survey file' and the file format specification. It gives step-by-step instructions on how to create the file for submission to the NSTS (using Excel). It also explains how to read the file which is returned from SchlumbergerSema. This document should be read in conjunction with the guidance manual for conducting a patient survey within the NHS Patient Survey Programme, produced by the Survey Advice Centre: Children and Young Patient Surveys Guidance.
