| Overview: | This data entry spreadsheet (MS-Excel) can be downloaded and used to prepare the data file for submission to the Patient Survey Advice Centre. The 'Data' sheet contains column headings for all the required fields which should be submitted (as a single 'worksheet'). For more information on preparing the data file, please refer to Section 15 of the Survey Guidance Manual. In addition, there is a series of sheets, corresponding to each section of the questionnaire, which automatically give Trusts a summary report of the data entered (frequency counts and percentages on each question). |
