| Overview: | This data entry spreadsheet (MS-Excel) can be downloaded and used to prepare the data file for submission to the Co-ordination Centre. The 'Data' sheet contains column headings for all the required fields which should be submitted (as a single 'worksheet'). For more information on preparing the data file, please refer to Section 17 of the Survey Guidance Manual. In addition, there is a series of sheets, corresponding to each section of the questionnaire, which automatically give Trusts a summary report of the data entered (frequency counts and percentages on each question). |
